What makes a good job interview question?

So you're going to interview someone for a job. What kind of questions do you ask?

Obviously, the interviewee needs to have a certain skill set in order to do their job, but beyond that how do you get at their personality, their motivators, what they would be like to work with?

I think where I'm landing is you want to ask questions like... "Tell me a time where you failed at your job. What was it like? What did you do?" or "What are things that you value in a team culture?"

Asking questions that are personal in nature and get to how they interpret a question versus something that is leading and seems to maybe have a right and wrong answer. Ultimately you don't want the person you're interviewing to just give you answers that you want to hear, because that's going to blow up later if it's not true for them.
